This form is for Interstate and Foreign Missionaries on Salary or Designated Funds who report to the office of Secretary-Treasurer of Missions.

Missionary or Sponsoring Church:
Please complete and submit early enough for the Secretary-Treasurer to receive it by the 20th and not later than the 25th of each month. These six days only shall missionaries be paid. Exceptions will be considered. The check will be sent to the sponsoring church of the missionary.
